An "elbow" is slang for a small tourniquet used by heroin addicts to raise the vein on the inner side of their elbow. By suggesting a price of $450 for the "elbow" alone, users of this drug are able to slip through legal loopholes when called into court on the matter. They will simply claim that they were buying the tourniquet - an legal act in all states but a few in the South East USA (where it is considered "jailable parapharnalia" - a vestige of legislation from the 80s when the local heroin problem spiked to extremely high levels for a few years). In reality the $450 will go to the purchase of a heroin "quid" equivalent to ~4.5 grams on the street. A "quid" of heroin can be taken in one of several ways - either powdered and snorted (a favored technique in the central USA), inserted into the rectal cavity for anal absorption (the blood vessels of the rectum are extremely close to the surface allowing for the rectal method to be extremely effective - this method is generally preferred by long-time addicts who are intent on maintaining a steady high), or mixed with a solution of saline and a coagulant like karo syrup and injected into the veins. As the suggestion has been made that this is an "elbow action," however, it is safe to assume that this user was referring to the injection into her veins. The 4.5 gram amount will be sufficient to keep an addict satisfied for several days. Intra-veinal injections are accomplished using needles such as those used for insulin by diabetics. Care must be taken by the heroin-user to wash his needles thoroughly prior to using them due to the dangers of accidental injection of insulin. Insulin is a powerful diabetic drug which interacts chemically with heroin to produce allanol, a chemical cousin of atropine (the active ingredient in belladona or deadly nightshade). Out here in Illadelphia top of the line manicured, cured, and cared for gear goes for 3500 to 4000 per elbow, and it's pretty much the same in the rest of the country.
